With the patch a few months ago to fix sparrow wall-breaching, it removed a lot of locations that you can glitch out of the map and explore. However, there's still quite a few that remain working, and new locations found as a result.
Below is a guide that will try to cover all of the currently working glitch locations that do not require sparrow wall breaching. These videos are quick and to the point, done on a Hunter with Bones of Eao, max agility and Triple Jump. I don't fully explore the areas and every nook and cranny, mainly to show how to get out of the map and immediate surroundings.
While a lot of these I found myself during ghost/fragment hunting (far too many hours searching for the new ones added), quite a few were found by others over time too. A lot of these came from memory or shown by friends, so full credit to the original finders in these cases, as I was not the original finder of all of these.
Most of these have been in the game since the beginning and have never been touched - hopefully it remains that way for people who like to explore, especially when new content becomes available. You will not get banned for doing any of these either.

EARTH:
Glitch Location: Patrol - The Steppes
Accessible Locations: The Steppes // Dock 13 // The Divide
Notes: This glitch can be accessed easily when loading into patrol. It will allow you to get on top of the building that takes you to The Divide, glitching through the area there.
Glitch Location: Patrol - The Breach
Accessible Locations: The Breach // Cosmodrome
Notes: This is done inside the very first area that you access in the game. You can go back through The Divide on patrol to enter here for the glitch. It allows you on top of The Breach, and you can path back to Cosmodrome. I died before exploring further, but might be able to get further into there.
Glitch Location: Patrol - Rocketyard / King's Watch
Accessible Locations: Rocketyard // King's Watch
Notes: You enter here by sprinting and crouch sliding. You'll be able to get to very few parts of Rocketyard, the main area being King's Watch here. You can explore the entire map of King's Watch.
Glitch Location: Patrol - Refinery / The Grottos
Accessible Locations: Refinery // Rocketyard // The Grottos // The Mine
Notes: This is only able to be done on the Hunter to push through the initial roof. You can then go back towards The Refinery and enter the room with the train in there, or you can go towards The Grottos and continue to The Mine. This glitch opens the map up quite a bit, and you may be able to get to more locations with creative jumping. Also allows you to "cheese" the Fallen S.A.B.E.R Warsat if you so wish (a very roundabout method though).
Glitch Location: Patrol - Mothyards / Lunar Complex
Accessible Locations: Mothyards // Lunar Complex // Skywatch
Notes: This glitch mainly lets you explore the area of the Lunar Complex area (the area between Mothyards to Skywatch). You can then travel to the Mothyards or Skywatch if you wish. You won't be able to get too far into either location, it might just load the transition. Mainly for Lunar Complex.
Glitch Location: Patrol - Forgotten Shore / Terrestrial Complex
Accessible Locations: Forgotten Shore // Terrestrial Complex // Skywatch
Notes: You must have loaded the "Terrestrial Complex" area and head back to the location before this will work. If your ghost says "Forgotten Shore" it may not work. This allows you to glitch out of the path that leads from Forgotten Shore to the Skywatch area. The same place you follow for the Omnigul strike.
Glitch Location: Cayde's Stash Mission - Devil's Spire
Accessible Locations: Devil's Spire
Notes: This one just lets you get out of the tower Devil's Spire in the Cayde's Stash mission to explore some of the areas. There's a hidden room you can enter, and you can head around the outside. Not too much to explore, but if you want to see the hidden room you can head here. Sword recommended for this, likely necessary.
MOON:
Note: A lot of the locations on the Moon will connect to each other in various ways. Most of them that do connect will meet up at the Circle of Bones, which essentially allows you to explore the entire underground tunnels through glitching.
Glitch Location: Patrol - Circle of Bones
Accessible Locations: Circle of Bones // Hall of Wisdom // Shrine of Oryx // The Summoning Pits // The World's Grave.. nearly everywhere in the underground tunnels.
Notes: The Circle of Bones glitch allows you to access essentially everywhere you can in the underground tunnels on the Moon. It just depends how dedicated and far you're willing to go. In this video I show two possible locations; one from the Circle of Bones through Hall of Wisdom to Shrine of Oryx. The other being to The Summoning Pits for the Phogoth Strike. This opens up the map quite a bit.
Glitch Location: Patrol - The World's Grave #1
Accessible Locations: The World's Grave // Circle of Bones // Chamber of Night
Notes: This glitch will allow you outside of The World's Grave from patrol, allowing you to explore the area there. You can go back to the Circle of Bones (which again opens up the entire map), or continue on to the Chamber of Night area. It will just load you into that area, making it accessible on patrol.
Glitch Location: Missions - The World's Grave #2
Accessible Locations: The World's Grave
Notes: There really isn't too much to this one. This door opens up for the "Lost to Light", "The Wakening" and "Chamber of Night" missions. Once in here you might be able to explore around further. I took a path that made me hit a dead end and didn't put too much further effort into it as there's locations around here to go further with ease.
Glitch Location: Missions - Chamber of Night
Accessible Locations: Chamber of Night
Notes: This one can be accessed on the "Lost to Light" and "The Wakening" missions. The entire roof up there is able to be jumped through, and you can do so from both sides. I chose the left as it's easier to land from that location. You can then go forward to the crystal room and be out of bounds there, and you might be able to head back into The World's Grave.
Glitch Location: Patrol - Hellmouth
Accessible Locations: Hellmouth // The Gatehouse
Notes: This one requires a sword to push through the barrier and make some of the further jumps. It allows you to get up on top of the Hellmouth area and where you begin the Crota's End raid. From here you can jump down the path you follow in the Phogoth strike, and if you so wish you can follow it further to the Circle of Bones and link up to everything.
Glitch Location: Taniks Strike
Accessible Locations: Dusk Warren // Temple of Crota
Notes: This is where the lift is on "The Shadow Thief" strike that takes you up to fight Taniks. In the room before the lift you can climb and get on top of the map. There isn't too much up there but a bunch of false ground. You can also jump down and backtrack to the Temple of Crota area and explore around there a bit. I assume you can do this when "Lost to Light" is the daily heroic as it opens up the area too.
Glitch Location: Crota's End Raid #1
Accessible Locations: Area on top of forming the bridge.
Notes: This one most people will know who used it to cheese the raid and snipe from up there. You can jump up to the rocks at the side and it allows you on top of the towers from where you enter that encounter. You can see outside of the map bounds from here and in places where you can't usually access, hence the inclusion.
Glitch Location: Crota's End Raid #2
Accessible Locations: On top of Crota boss room, backtrack through map.
Notes: This one allows you to get on top of the area where you fight Crota to essentially anywhere in that area. You can get to the second level of the room where you fight Ir Yut, or head all the way over the top and back to the bridge forming area again and on top of that area. Probably one of the highest locations in the game glitch wise, there doesn't seem to be a height barrier either.
VENUS:
Glitch Location: Patrol - Winter's Lair
Accessible Locations: Winter's Lair
Notes: This one lets you out of the map of the Winter's Lair, the first area you would have encounter enemies in the Archon Priest strike. There isn't all too much to explore, you may be able to force a transition further - I didn't attempt it too much. Mainly just the Winter's Lair and above/below it.
Glitch Location: Patrol - The Citadel
Accessible Locations: The Citadel
Notes: Not too much to this glitch either. It allows you on top of the map and on the buildings next to The Citadel. You can jump down a little and there's some ledges you can jump to, but apart from that an invisible barrier prevents much further.
Glitch Location: Patrol - Outside Vault of Glass
Accessible Locations: Waking Ruins // Vault of Glass
Notes: Outside the door of the Vault of Glass there's a hole that you can jump into and out of the map. It lets you under the entrance of Vault of Glass and some ledges you can jump to. If you get creative with sparrow/swords/Bladedancing you can go further and around the area.
Glitch Location: Patrol - Inside/On Top Vault of Glass
Accessible Locations: The Vault of Glass
Notes: You can use your sparrow to enter the door of the Vault of Glass, and immediately to your left there's a hole you can jump out of. Once out of here, you can be on top of the Vault of Glass and explore around the raid and the outside areas. There's invisible barriers preventing you from entering the raid location, but there's quite a bit you can explore using this glitch and see from different perspectives.
Glitch Location: Patrol - Endless Steps
Accessible Locations: Endless Steps
Notes: Okay, this is probably the most useless out of map glitch I've found... but it's still out of map so I included it. These two spots allow you under the Endless Steps at certain locations. There isn't too much to explore from there, you might be able to get creative and find something. I didn't bother personally.
Glitch Location: Patrol - Endless Steps / Echo Chamber
Accessible Locations: Endless Steps // Echo Chamber
Notes: This one allows you to enter the Echo Chamber on patrol, for both Xbox and Playstation. This is mainly used for Xbox users so that they can obtain the dead ghost from the Playstation Exclusive strike. Bit of a fun jumping puzzle to get there, and lets you explore the Echo Chamber without the barriers.
Glitch Location: Mission - The Archive
Accessible Locations: The Archive // Hall of Whispers
Notes: This is from the mission "The Archive" and allows you to exit the map from the final room in there. You can then get on top of the map and climb back to the Hall of Whispers. You may be able to go further, I found barriers around the place but you could get creative.
MARS:
Glitch Location: Patrol - Dust Palace Strike Area
Accessible Locations: The Hollows // Dust Palace Strike
Notes: This glitch allows you to glitch into the entire area you follow for the Dust Palace strike (the Psion Flayers). You can access this on patrol and climb the tower all the way to the final boss room and enter there. There's a lot to explore here, I show the path to the boss room.
Glitch Location: Patrol - The Buried City / The Black Garden
Accessible Locations: The Buried City // Freehold Station // Tharsis Junction // The Black Garden
Notes: This glitch allows you to enter The Black Garden from patrol, glitching on top of Tharsis Junction to get there. If you follow the path you can head to the Black Garden, and enter the zone if you drop down into the area.
Glitch Location: Patrol - The Black Garden #1
Accessible Locations: The Black Garden
Notes: This can be accessed from when you first enter The Black Garden on the mission, or if you used the glitch earlier from The Buried City you can get here on patrol. There's a hole in the wall above the rocks immediately to your right, and from here you can get above and explore. I just go to some basic locations in here, the next Black Garden glitch I show further.
Glitch Location: Patrol - The Black Garden #2
Accessible Locations: The Black Garden
Notes: This can be accessed from when you The Black Garden on the mission, or if you used the glitch earlier from The Buried City you can get here on patrol. You proceed forward until the first room that has light as shown in the video, and you climb to the very top of that window. From there you crouch forward, and you can either crouch slide or sword through. After that you can climb on top of The Black Garden map, and explore essentially everywhere. There's some floating grass texture shown too, but it has a barrier preventing you to get there.
Glitch Location: Patrol - Valley of the Kings / Valus Ta'aurc Strike
Accessible Locations: Valley of the Kings // Cerberus Vae III
Notes: This is the ship you enter just before you fight Valus Ta'aurc/Rockets McDickface in the Cerberus Vae III Strike. Just outside the ship there's a hole underneath it you can jump into. From there you can climb on top and head towards the rooms leading to the boss, and on top of the boss room itself.
DREADNAUGHT:
Glitch Location: Patrol - Hull Breach #1
Accessible Locations: Hull Breach
Notes: From when you first enter patrol, you can turn around and jump on the pillars below you. It allows you into a small area that's under the Hull Breach map. Isn't too much to explore there, but it's under the map regardless.
Glitch Location: Patrol - Hull Breach #2
Accessible Locations: Hull Breach
Notes: This is another location in the Hull Breach that's pretty simple to get into. When you first enter the cabal ship the glitch is just above you. You'll need to crouch and slide a bit to get through the gap. It allows you around the initial area of the ship - you may be able to climb up further and around.
Glitch Location: Patrol - Cabal Ship Dantalion Exodus VI
Accessible Locations: Datalion Exodus VI // Hull Breach // Mausoleum // Hall of Souls // The Bridge
Notes: This one opens up the map quite a bit and allows for exploration. From here you can hit pretty much every part of the Dreadnaught ship with creative jumping, but it will take some time and effort. I just showed the basic way above The Bridge area and what it looked like there. But depending on where you head, you can hit most of the map.
Glitch Location: Patrol - Mausoleum
Accessible Locations: Mausoleum
Notes: This one allows you to get under the Mausoleum area, and then you can drop down even further and explore some more. It remains inside the Mausoleum area, but you drop down pretty far and can see quite a bit.
Glitch Location: Patrol - Shield Brothers Strike Area
Accessible Locations: The Bridge // The Narthex // Shield Brothers Boss Area
Notes: You can get into this room initially on patrol by jumping over the door as shown in the cabal ship video. From here, once you enter the room where the Ultra Knight spawns, you can jump out of the map and continue above it. You can load into the boss fight area where you fight the Shield Brothers, and the room where they come out of is also no longer blocked and you can look in there too.
Glitch Location: Mission - Regicide Under Oryx
Accessible Locations: Trenchway, under where you fight Oryx.
Notes: This glitch allows you to get underneath the room where you fight Oryx in the Regicide mission. You need to climb the side of the statue and get into here. It can be a little tricky, and there isn't too much underneath there.
Glitch Location: King's Fall Raid #1 - Totems
Accessible Locations: Basilica (Totems/Warpriest area) // Portico (Ships)
Notes: This allows you to get out of the map on the Totems encounter, and explore around there. You can also backtrack to the ships from here and explore there too, which is the path I took here. There's a lot more you can explore, but I didn't want to go for too long. I would definitely recommend doing this on a Titan instead of a Hunter too. I did it on a Hunter to show a method to get up there with the class (albeit can be difficult this way), whereas a Titan with Higher Jump will have much more ease doing this glitch.
Glitch Location: King's Fall Raid #2 - Before Daughters/Oryx
Accessible Locations: Outside the door before Daughters/Oryx at the jumping puzzle.
Notes: This allows you above the door at the end of the jumping puzzle/dicks room. You can't get much further than this, but it does make you appear like you've gone through the door while it is locked.

Notes: This is the ship you enter just before you fight Valus Ta'aurc/Rockets McDickface in the Cerberus Vae III Strike. Just outside the ship there's a hole underneath it you can jump into. From there you can climb on top and head towards the rooms leading to the boss, and on top of the boss room itself.
DREADNAUGHT:
Glitch Location: Patrol - Hull Breach #1
Accessible Locations: Hull Breach
Notes: From when you first enter patrol, you can turn around and jump on the pillars below you. It allows you into a small area that's under the Hull Breach map. Isn't too much to explore there, but it's under the map regardless.
Glitch Location: Patrol - Hull Breach #2
Accessible Locations: Hull Breach
Notes: This is another location in the Hull Breach that's pretty simple to get into. When you first enter the cabal ship the glitch is just above you. You'll need to crouch and slide a bit to get through the gap. It allows you around the initial area of the ship - you may be able to climb up further and around.
Glitch Location: Patrol - Cabal Ship Dantalion Exodus VI
Accessible Locations: Datalion Exodus VI // Hull Breach // Mausoleum // Hall of Souls // The Bridge
Notes: This one opens up the map quite a bit and allows for exploration. From here you can hit pretty much every part of the Dreadnaught ship with creative jumping, but it will take some time and effort. I just showed the basic way above The Bridge area and what it looked like there. But depending on where you head, you can hit most of the map.
Glitch Location: Patrol - Mausoleum
Accessible Locations: Mausoleum
Notes: This one allows you to get under the Mausoleum area, and then you can drop down even further and explore some more. It remains inside the Mausoleum area, but you drop down pretty far and can see quite a bit.
Glitch Location: Patrol - Shield Brothers Strike Area
Accessible Locations: The Bridge // The Narthex // Shield Brothers Boss Area
Notes: You can get into this room initially on patrol by jumping over the door as shown in the cabal ship video. From here, once you enter the room where the Ultra Knight spawns, you can jump out of the map and continue above it. You can load into the boss fight area where you fight the Shield Brothers, and the room where they come out of is also no longer blocked and you can look in there too.
Glitch Location: Mission - Regicide Under Oryx
Accessible Locations: Trenchway, under where you fight Oryx.
Notes: This glitch allows you to get underneath the room where you fight Oryx in the Regicide mission. You need to climb the side of the statue and get into here. It can be a little tricky, and there isn't too much underneath there.
Glitch Location: King's Fall Raid #1 - Totems
Accessible Locations: Basilica (Totems/Warpriest area) // Portico (Ships)
Notes: This allows you to get out of the map on the Totems encounter, and explore around there. You can also backtrack to the ships from here and explore there too, which is the path I took here. There's a lot more you can explore, but I didn't want to go for too long. I would definitely recommend doing this on a Titan instead of a Hunter too. I did it on a Hunter to show a method to get up there with the class (albeit can be difficult this way), whereas a Titan with Higher Jump will have much more ease doing this glitch.
Glitch Location: King's Fall Raid #2 - Before Daughters/Oryx
Accessible Locations: Outside the door before Daughters/Oryx at the jumping puzzle.
Notes: This allows you above the door at the end of the jumping puzzle/dicks room. You can't get much further than this, but it does make you appear like you've gone through the door while it is locked.
